We offer a simple four-step process.
1. Contact Us
Contact our office to speak with a member of clinical leadership. Please have your insurance information available to share with us.
2. Receive Intake Packet
If we are in network with your insurance, we will send you a Clinical Services Intake Packet. We also offer private pay options.
3. Return Packet with Required Material
Complete and return the intake packet along with required documentation, including diagnosis, referral, and any assessments or evaluations. Remember, intake and required documents must be provided to be placed on our waiting list.
4. Receive Next Steps
After reviewing your information, a member of our clinical leadership team will give you an estimated start date and go over next steps.
